Plans designed to fit your needs
The Montana Logging Association offers an association Group Health Insurance program fully insured through Blue Cross/Blue Shield of Montana (BCBSMT). To obtain a program rate quote, call the MLA office (406-752-3168).
Our program includes three different health insurance options:
- Blue Choice - The Blue Choice plan is a "point-of-service" health care plan with benefits that provide very attractive coverage with no deductible and preventive health benefits when you go to your Personal Care Physician (PCP). This product is unique because you may choose to go to any doctor you want and still have comprehensive coverage for your care. Levels of payment are determined by the participating status of the professional provider you choose to go to.
- Comprehensive Major Medical 50/50 - The Comprehensive Major Medical 50/50 plan has a $1,000 individual and $2,000 family aggregate deductible that must be met before the plan pays 50% of allowable fees. The deductible is waived for these services: BCBSMT participating professional provider services, Well Child Care, Home Health Care, Hospice, Mammograms, Chiropractic, Diabetic Education, and Routine Newborn services. There is also a $300 preventive benefit with this plan.
- HDHP Montana - This is a high deductible plan with a $2,500 individual deductible and $5,000 family deductible. The entire family deductible (for two-party and family coverage) must be satisfied before benefits are paid on any one family member. After the deductible is met, the plan pays 100% of allowable fees. There is a $300 preventive benefit with this plan. It qualifies, under the federal requirements for a high deductible health plan, to have a Health Savings Account associated with it.
Dental and vision insurance is also available through the association group health program with Blue Cross/Blue Shield of Montana. MLA Services, Inc. is able to write merit group health insurance plans (for employers with more than 100 employees) and community group plans (for employers with less than 100 employees) with either Blue Cross/Blue Shield of Montana or New West Health Services. We can also help you implement a flexible spending account (FSA), health reimbursement account (HRA), and health savings account (HSA).
